A health care representative designation is a document that appoints a person to make medical decisions on your behalf if you are ever unable to do so on your own. This document does not grant any powers to anyone until a physician has certified that you are unable to make decisions on your own behalf. The powers which you have granted to your health care representative are detailed in this document. Your health care representative should present this document to any medical providers who are providing care for an incapacitated person. You should notify your health care representative and any successor health care representatives that you have entrusted them with this responsibility and let them know where they might find the documents which will help them exercise their powers according to your wishes.
